BORROWER'S CARDS
To obtain an Elmont Public Library card TWO items of identification with name and address are required as follows:
Drivers license or non-driver's ID or Car registration
Current utility bill (within last three months)
Tax bill (within last three months)
Voter registration
Bank and/or credit card statement (within last three months)
Official mail (government, state, etc. within last three months)
Race Track badge and letter from dormitory supervisor
In lieu of one (1) of the above identifications, a written, notarized affidavit from the person with whom the applicant resides will be accepted, providing that the person is himself or herself a registered borrower.
If the applicant is under 14 years of age, identification must be provided by the legal guardian. If the applicant is between the ages of 14 years and 18 years, one (1) form of school identification or report card (with applicant's name and address) may be presented at time of application.
Renewal or application for a lost card for a patron with an existing account requires one form of identification with card holder's name and address to be presented at the time of renewal or application for a lost card.
Juvenile Cards - are issued through eighth grade. Seventh and eighth graders' are allowed to take out adult fiction and non-fiction books for use with school assignments.
Property Owner - a borrower's card may be issued for one year only to a non-resident individual who pays Elmont Union Free School District taxes. Upon presentation of a current school tax bill as proof of taxpaying status, the individual extended borrowing privileges shall include the single or major stockholder of a tax-paying corporate entity or a stockholder designated by the Board of Directors of the corporation as the one to be extended borrowing privileges.
Non-Residents (Queens County) - borrower's cards issued for one year for a fee of $145.00.
Teachers - upon proof of current employment as a teacher in the Elmont Union Free School District, a borrower's card will be issued for the current year only, and will expire on the last day of school. This must be renewed each school year. In addition, Library cards are available to teachers in the Elmont High School and Sewanhaka High School.
Members of Clergy - whose ministries are in the Elmont Union Free School District or the Franklin Square Funding District. Upon proof of employment in a district parish, a borrower's card will be issued for one year; to be renewed annually.
East Franklin Square, North Valley Stream Library Funding District Residences - As of January 1, 2007 both the East Franklin Square Library Funding District and the North Valley Stream Library Funding District residences with proper identification (described above) will be issued Library cards that will expire on December 31, 2010.
